You can fax the requested records to our team. Include a cover sheet so we can match them to the right requests.

1. Include the request reference from your letter on the cover sheet.
2. List the account references and dates of service covered by your records.
3. Keep your fax transmission receipt for your records.

Our team will use the packet reference to match your fax to the correct requests.
